How FORMA handles the details you send through the booking form on this site.

What the form asks for

To book a call, the form asks for your name, your email, your business name, your type of business and your province. Your phone number, website and message are optional.

How your details are used

Chris uses your details to reply to you, set up your call and prepare the check of your website and your roadmap. Your details aren’t sold.

Form submissions are stored with Netlify, the company that hosts this site, where only Chris can see them.

Who else sees them

Your form details go to Chris Gardner, who works with LocaliQ. Chris consults. If you go ahead with services, your agreement is with LocaliQ.

The tools on this site

Nothing you type on this site is sent anywhere until you send the booking form.

Cookies and analytics

This site sets no cookies and runs no analytics or ad tracking.

If analytics are added later, such as Google Analytics 4 to count visits, this page will be updated first, and a notice will say if cookies are used.

How long details are kept

Your details are kept only as long as they’re needed to follow up with you and to run any services you sign up for. You can ask for them to be deleted at any time.

Your choices

You can ask to see the details you sent, and ask for them to be corrected or deleted. Email chris@formagrowth.ca.

Changes to this page

If this page changes, the date at the top changes too.
